Does Sparing Xanthe Change Greifberg Castle?

Does Xanthe Help You Fight Brencis?
Yes. Following Xanthe’s alliance route can give Coen additional help during the final confrontation with Brencis.

Rather than removing another powerful vampire and continuing alone, you turn one of Brencis’ own Court members against him.
That makes sparing Xanthe particularly attractive if your priority is making the final stages of the game easier.
Do You Get Blood Surge If You Spare Xanthe?
No. Sparing Xanthe means you do not drain her blood, so you do not obtain Blood Surge from her.
Blood Surge is the major gameplay reward associated with drinking Xanthe’s blood after defeating her.

What Happens to Chariklea If You Spare Xanthe?
Chariklea remains with Xanthe, and Xanthe continues trying to preserve her daughter’s life.
This is where the spare choice becomes much less straightforward.
Xanthe’s methods for sustaining Chariklea involve abducting and killing other women. Sparing Xanthe therefore does not simply produce a conventional “good” outcome where everyone survives without consequences.
The later story makes clear that Xanthe continues her horrific methods to sustain her daughter.
So the choice is deliberately uncomfortable: helping Xanthe gives Coen a useful ally, but it also allows her actions to continue.
Can You Spare Xanthe If You Find Her Early?
No. The early Xanthe encounter does not give you the same spare-and-ally option as The Cycle of Love route.
If you find Xanthe early at Svartrau Cathedral, you can confront her without going through the normal Court Anger progression.
However, this shortcut does not provide the full post-fight choice available through The Cycle of Love. The early encounter lets you kill or drain Xanthe, but not form the alliance.
